In order to obtain a copy of your hospital medical record, please contact the Medical Director or complete a request form. Representatives are available Monday through Fridays from 8 a.m. to 4 p.m. at the phone number +233.20.630.1342

• Fees and delivery expectations

  Patients should allow 10 business days to process the request. Medical records stored off-site are to be released within 14-21 days after receipt of the request. All records are preferrably handed over in person to patient. There is no fee if the record is being released to a doctor or healthcare provider for continuity of care.

• Policies & procedures

  It is the obligation of COY Paediatrics to protect the confidentiality of the patient's medical record. Any information contained in the medical record is confidential and protected. Therefore, patient information may only be released upon receipt of appropriate patient authorization, valid subpoena or court order.
  
  Parents or legally appointed guardian, may obtain a copy of a minor's record upon receipt of a written request. When a minor was treated for sexually transmitted disease, birth control treatment, drug/alcohol abuse treatment, HIV or mental illness, the records can only be released with the minor's assent.
  The records of a deceased patient may be obtained by a qualified person or personal representative upon receipt of a written request.
